Stanford University researchers tested five leading AI therapy chatbots to examine how they respond to individuals with different mental health conditions. The results revealed a troubling trend: the bots showed more stigma toward users presenting symptoms of disorders such as schizophrenia and alcohol dependence compared to those describing depression. For example, when prompted with hypothetical case studies, the bots were more likely to label individuals with certain diagnoses as potentially violent or socially undesirable.

This bias mirrors harmful real-world stereotypes and undermines the core principles of mental healthcare, where nonjudgmental support and empathy are critical. The study emphasizes that AI therapy chatbots, even those built on cutting-edge large language models, can perpetuate or even amplify societal stigma. That’s especially concerning given their growing popularity among users seeking affordable or anonymous mental health support.

Therapeutic Shortcomings of AI Chatbots

Beyond stigmatization, the study also found that these chatbots fail to meet key therapeutic standards. Evaluating the bots using guidelines for ethical and effective human therapy, researchers found that AI-driven responses often lacked nuance, empathy, or proper boundaries. Some bots gave advice that could be interpreted as dismissive, overly simplistic, or even harmful when addressing sensitive issues such as suicidal ideation or trauma.

Nick Haber, senior author and assistant professor at Stanford, noted that while chatbots are increasingly seen as "companions, confidants, and therapists," they lack the emotional intelligence, clinical judgment, and situational awareness required for responsible mental healthcare. This distinction matters: vulnerable individuals may overestimate the capabilities of these digital assistants, mistaking chatbot feedback for professional guidance.

Why AI Therapy Chatbots Still Aren’t Ready to Replace Human Support

The findings serve as a critical reminder that AI therapy chatbots cannot and should not replace licensed mental health professionals. While they may offer value as supplemental tools—for example, through mood tracking or cognitive-behavioral prompts—they are not a safe stand-in for trained therapists. Relying on AI for primary care in this area risks overlooking complex human emotions, diverse cultural contexts, and the unpredictable nature of mental health crises.

Furthermore, the assumption that future models will automatically improve these shortcomings may be overly optimistic. According to lead author Jared Moore, even newer, more powerful language models exhibited the same stigmatizing behaviors as older versions. “Business as usual is not good enough,” he warns, suggesting that developers must go beyond scaling up models to fundamentally rethink the ethics, oversight, and training data behind therapy bots.
